School Health Services

All students are required to have an updated emergency form completed yearly:
If your student will be taking medication while in school, the below forms are required.
No student should be carrying their own medication unless cleared by the school nurse.
**All medications should be delivered to the nursing office by a parent or guardian.
Medications need to be in their original bottle with a prescription label.
