School Health Services
School Health Services fosters the growth, development and educational achievement of Massachusetts' students by promoting their health and well-being. It monitors health status and identifies and addresses the unmet needs of students, families and school personnel.
Required Forms
All students are required to have an updated emergency form completed yearly
Medication Policy
If your student will be taking medication while in school, the below forms are required. No student should be carrying their own medication unless cleared by the school nurse.

**All medications should be delivered to the nursing office by a parent or guardian. Medications need to be in their original bottle with a prescription label.
